DEPOSIT: High-Risk Monitor Package (21 Days) | Private Postpartum Nurse Care

This product represents a clinical nursing service deposit for RN-led postpartum monitoring and care coordination. Services are delivered after clinical intake approval. Deposits reserve clinical capacity and are non-refundable. Remaining balance is due prior to service initiation.

The High-Risk Monitor Package provides RN-led clinical monitoring and care coordination for postpartum mothers who require closer medical observation during recovery. Only available in HOUSTON, TX.

This package supports early detection of complications and structured communication with your obstetric provider during the critical weeks following discharge.

This is a clinical postpartum nursing service.

What This Package Includes

• Three (3) in-home Registered Nurse visits 

• Three (3) scheduled virtual RN check-ins

• Ongoing postpartum nursing assessments

• Blood pressure monitoring and education (when indicated)

• Blood glucose monitoring support and education for recent gestational diabetes

• Incision or perineal assessment and recovery monitoring

• Medication adherence review and safety education

• Threshold-based escalation and provider communication

Appropriate For

• Advanced maternal age with additional risk factors (35+)
• High Risk postpartum risk
• Postpartum blood pressure concerns or history of preeclampsia

• Recent gestational diabetes

• Cesarean delivery or complex recovery
  

• Early hospital discharge, or the patient who had to return to the hospital and has since been discharged with ongoing clinical concerns

• Mothers who desire structured clinical follow-up before their postpartum visit

Important Information

• Services are delivered by a licensed Registered Nurse 

• This service does not include diagnosis, prescribing, or treatment decisions

• As we partner with them, our OB or midwife remains the diagnosing and treating provider

• Emergency symptoms require immediate medical attention (call 911 or go to the nearest emergency department)

This product requires a non-refundable clinical deposit to reserve RN care capacity. Remaining balance is due prior to the first in-home visit.
